Legacy Athletic Handbook

Be aware that you will probably need to bring the UPDATED physical forms for the doctors to sign. See the attached forms or you can go on our website and click the athletics tab and select "forms". Please remember if you are a 9TH-12TH grade athlete you will also need an EKG on file if you haven't already done so in your high school career. 

The following forms MUST be completed and submitted to the coach to be eligible to participate. Athletes may not practice until they are in. 

Your Doctor must sign, stamp, and fill out the EL2, EL3 and EKG form! Please fill out the paperwork below.
